Social Worker (BSW) Full Time

​Job Summary: Assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet clients’ psychological, social, environmental, and physical needs.  Other responsibilities include evaluation of service efficiency and excellence, provision of social support, crisis support, and client education. Actively participates and facilitates tracking of cost, resource management and quality improvement parameters. Assists in screening, third-party payor authorization, and discharge activities providing linkages to health and wellness resources across the health-care continuum. Uses social services expertise and problem solving skills to achieve optimal social, psychological and resource outcomes within effective time frames.  Promotes interdisciplinary collaboration with a team approach. Provides discharge planning services. Participates in Risk Management. Makes indicated client referrals. Supports physicians in meeting the social needs of clients.
Essential Duties and Responsibilities include the following.  Other duties may be assigned.

Evaluates each situation prior to action to ensure appropriateness of direction. Uses sound investigational and analytical skills consistently, effectively resolving issues and proactively initiating process improvements as indicated.

2.Displays a positive focus and effectiveness in building working relationships with the interdisciplinary team members. Consistently: Displays an ability to build positive relationships with medical staff. Serves as an ambassador for Case Management, promoting education as to our role, serving others as a resource, and seeking guidance / giving direction as needed. Establishes and maintains a collaborative relationship with members of the interdisciplinary care team
3.Participates in department / organizational educational requirements. Participates in mandatory departmental training and education as related to particular venue of medical management. Completes competencies in a timely manner and with required minimal scores
4.Demonstrates responsibility and accountability for own professional practice. Demonstrates responsibility and compliance in documentation requirements. Demonstrates commitment to professional improvement and personal growth
5.Participates in meetings, reports, and other administrative or collaborative activities as requested. Meets agreed deadlines and goals. Performs data collection, analysis, and report generation as assigned to facilitate performance improvement. Assists and facilitates assigned meetings, PI teams, or in services as indicated
6.Demonstrates understanding of roles of governance committees, administrative officers, and various focuses of practice of the department. Serves as a resource for others in areas of Case Management focus. Demonstrates understanding of leadership roles and how to appropriately access the Case Management system. Appropriately assists or refers individuals needing Medical Management support. Demonstrates working knowledge of departmental policies and procedures
7.Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, specific job related hazards, and national patient safety goals.

9.AGE APPROPRIATE SKILLS AND KNOWLEDGE: Demonstrates the knowledge and skills in addressing case management requirements/protocols, as described in departmental policies and procedures encompassing all age levels

Key Accountabilities/competencyKey Accountability

1.Performs psychosocial assessment for patients to determine social service and discharge needs.
2.Communicates with physicians, care team, patient, family and significant others regarding plan of care and assist in development of realistic goals.
3.Implements effective and appropriate psychosocial intervention Maintains current knowledge of resources available within the community maintains and provides patient/family with resource material.
4.Documents all Social Work activities and interventions in accordance with JCAHO, State, and organizational standards and policies, maintaining confidentiality of patient /physician personnel information.
5.Communicates and documents pertinent information in the patient’s medical record in a clear, concise, and timely manner.
6.Documents and reports to appropriate parties UM/CM and Case Management Drives.
7.Documents reported cases involving legal issues, Child/Adult Protective and Domestic Violence reports, criminal behavior or other high-risk issues
8.Accurately determines type of assistance setting resources necessary for the patient/family and provides appropriate referral and/or linkage to assistance.
9.Provides support with Advance Directives, guardianship issues, and barriers to appropriate care. 
10.Assists patient/family with adjustments, grieving, and end-of-life issues, including referrals for pastoral care, hospice, and community area support services. Assist patient/family, CM, and care team by facilitating post-acute care arrangements (i.e., SNF, Hospice, Home Care, IRF, LTAC).
11.Provides social and emotional support, crisis intervention, problem-solving strategies and client education. Coordinates appropriate referrals
12.Monitors physician/care team provider’s progress notes and orders.
13.Discusses recommendations regarding admission status and discharge barriers.
14.Collaborates with UM/CM, team leaders and other members of the health care team to provide for psychosocial assistance or optimum post-discharge services, including performance and outcomes improvement activities.
15.Effectively facilitates patient/family and team care conferences, demonstrating positive communication skills in all forums. Assesses and reports in accordance with State, Federal Mandates and organization policies regarding child and adult abuse, neglect, and domestic violence
16.Identifies needs of patient/family related to psychosocial, socioeconomic, medical, cultural, spiritual and environmental issues.
17.Explore financial issues and provide input with the development of a realistic and effective plan of care.
18.Consistently ensure patient/family’s holistic needs are identified and addressed and information relayed to care team.
19.Assess patient/family’s ability to participate in formation of the Plan of Care, Develop alternative interventions as identified (i.e., Power of Attorney, guardianship, health care surrogate, next of kin). Includes patient/family in development of plan of care and discharge coordination

Minimum Education, Experience, Training, and Licensures Required: Bachelor’s degree in Social Work from an accredited program is required. Licensure in Social Work in the state of Kentucky is required. Certification in area of expertise preferred. EXPERIENCE: Two years in a hospital setting or three years experience in casework. Must have experience in group technique and discharge planning. Computer Literacy. Excellent Communication skills, written and verbal. Excellent Customer Service and problem-solving skills.
